To complete our review process we need the items listed below. Your immediate response will expedite the process. If a listed document does not apply to your family, please disregard.

1. Application completed by each adult; signed and dated
2. Profit & Loss (if self-employed) prepared by a CPA or tax service along with business card
3. Profit & Loss self-employment affidavit
4. 3 most recent pay-stubs in consecutive order (look at period ending)
5. If applicable request quarterly statement for life insurance/retirement/pensions
6. Social security award letter (most recent)
7. Support letters ex: family income support (notarized)
8. TANF award letter
9. Attorney General/child support
10. No alimony child support affidavit
11. Copy of divorce decree/judgments
12. Estrangement affidavit
13. Texas workforce printout if unemployed
14. Zero income affidavit (must be attached along w/ TWC printout)
15. Verification of employment form (as needed)
16. 3 month bank statements (dated, stamped/or signed)
17. If required; detailed letter explaining bank deposits over $100.00
18. Income taxes 2005 & 2006
19. W2'^s for 2005 & 2006
20. Income tax affidavit
21. If applicant did not claim children; need notarized letter of explanation who claimed children, and why
22. Sex Offender
23. Credit Report
24. Id'^s and social security cards for everyone listed on application
25. Student verification form
26. Financial aid award letter (must be obtained from financial aid office)
27. Tuition statement; must be original, Not computer generated

If application is reviewed and additional paperwork is required the application will go into phase 2 status.

If the above items are not received in our office within 5 days your application will be withdrawn. You must call the office before the 5 days in the event that the paperwork will take longer to receive.
